Full time Case Manager position available. Case Manager will work as part of multidisciplinary team providing integrated whole-person system of care for individuals with serious mental illness or co-occurring mental and substance use disorders living with or at risk for HIV and Hepatitis in Los Angeles County. Duties include but are not limited to: Facilitate support groups; Serve as advocate for participants and assist them in accessing all needed services; Participate as a member of the Care Coordination Team and attend case conferences; Develop supportive relationships with participants and assist them in developing life skills. Requirements include at least a High School diploma or GED. Bilingual/Spanish is highly desirable.
